Camp Info and Paperwork

If you are accepted to an AuSM camp during the lottery process, it will be your responsibility to also submit proof of a medical exam for your camper within one year of start of your camp session. The easiest way to submit this proof is to print the Physician’s Signature Form (link below), write the camper’s name and date of birth on the top of the page, and bring the sheet to your doctor’s office or clinic for their verification and signature.
